The role will involve CHW functions such as A Community Health Worker (CHW) is a liaison and provides cultural mediation between the health care system, social services, and the community. A CHW is a trusted member, and has a close understanding of, the ethnicity, language, socio-economic status, and life experiences of the community served. A CHW assists people to gain access to needed services and builds individual, community and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, navigation and follow-up, community health education and information, social support, advocacy, and encouragement in participation of clinical screenings to support BOTH McKesson employees and to build McKessons presence with community agencies and partners in the local area. The candidate should be willing to travel locally and be flexible with their hours in order to accommodate different distribution center shifts. The CHW will meet with employees to assess and address non-clinical resource and healthcare needs as well as hold educational sessions, organize volunteer events. In the community, the CHW will meet with local organizations to build McKessons presence as well as new programs that can support the community. Minimum Job Qualifications:Degree or equivalent and typically requires 2+ years of relevant experience.Critical Skills/Experience:<ul><li>2+ years of relevant experience as a Community Health Worker</li><li>Proficiency in Microsoft Office Products (Outlook, Excel, Word, Teams, and PowerPoint) required</li><li>Must have exceptional communication skills</li><li>MUST be able to speak to Providers and Other Healthcare Professionals</li></ul> Additional Experience <ul><li>Experience with community-based outreach</li><li>Experience in health education and/or addressing health literacy</li><li>Extensive oral presentation skills</li><li>Successful completion of an approved CHW training program and related certification preferred. </li></ul></div>Working Conditions:Environment (Office, warehouse, etc.) –<ul><li>Traditional office environment within distribution centers located in KY.</li></ul>Physical Requirements (Lifting, standing, etc.):<ul><li>Based out of the distribution center, performs computer-based work, in-person work is required, travel will be approximately 75% of duties. Must be willing to travel to Louisville, Shepherdsville Kentucky and surrounding areas.</li></ul>Must be authorized to work in the US.
