Must be able to speak, read, and write in both languages without limitations or assistance. See Additional Information on testing.</li><li>2 or more years of case management experience.</li><li>Experience utilizing Microsoft Word, Excel and Outlook.</li><li>Experience with electronic information applications/software programs.</li></ul> Preferred Qualifications<ul><li>Certified Case Manager (CCM). </li><li>Masters degree in a health or social service-related field.</li><li>Prior experience with Medicare &amp; Medicaid recipients.</li><li>Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems.</li><li>Experience with health promotion, coaching and wellness.</li><li>Knowledge of community health and social service agencies and additional community resources.</li><li>Experience with case management, discharge planning and patient education for adult acute care.</li></ul>Additional Information<ul><li>Workstyle: remote, work from home.</li><li>Work Location: must reside in Illinois.</li><li>Typical Work days/hours: Monday – Friday, 8:00am – 5:00pm CST, occasional flexibility in scheduling may be required to accommodate client needs.</li></ul>Language Proficiency TestingAny Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.
