Job DescriptionThis position represents an amazing opportunity for a nurse practitioner (APRN)  with a big heart and entrepreneurial spirit who is passionate about chronic disease management.   In this role you will be working in a next generation virtual cardiometabolic clinic that dramatically expands access to medications for Americas most vulnerable seniors.  The clinic focuses on optimizing medication adherence for Devoted Health members with hypertension, hyperlipidemia and diabetes who are at risk of running out of their medications.You will serve as the prescriber in our medication renewal service that we call “Medications on Time” and will also work closely with our clinical pharmacy guide time that comprises over 90 pharmacy technicians who reach out and support members who are approaching or have become nonadherent to their medications. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits as well as managing asynchronous prescription refills for patients.  The cardiometabolic clinic is one of several of Devoteds virtual specialty care programs that are designed as  “micro centers of excellence” that deliver highly tailored, specialized care to patients with  specific chronic conditions.  Responsibilities will include:<ul><li>Conduct  focused and thorough assessments of patients with conditions that impact cardiometabolic health including hypertension, hyperlipidemia, coronary artery disease, and diabetes who are running out of medications.   We expect that 40-60%% of your clinical effort will be performing virtual visits. </li><li>Formulate accurate diagnoses and develop individualized treatment plans for patients with cardiometabolic conditions, including medication management, lifestyle modifications, and monitoring recommendations.</li><li>Manage a refill inbox for patients with cardiometabolic conditions in accordance with established protocols and guidelines.  We expect that 40-60% of your clinical effort will be spent managing a prescription refill queue. </li><li>Identify and evaluate risk factors, comorbidities and possible contraindications for treatment.</li><li>Provide counseling on medication adherence, potential side effects.</li><li>Collaborate closely with other members of the care team including PCPs, endocrinologists, cardiologists and other Devoted team members including pharmacy, clinical nursing, social work, as  well as interfacing with family members and caregivers as needed  to coordinate  holistic care for the member, to ensure continuity of care and deliver a collaborative care plan.</li><li>Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface. </li><li>Maintain accurate and up-to-date patient medical records, ensuring compliance with relevant legal and ethical guidelines.</li><li>Participate in quality improvement initiatives  and ongoing professional development to stay current on best practices and advancements in cardiometabolic care.</li><li>Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.</li></ul>Attributes to success:<ul><li>Skilled nurse practitioner with thorough understanding of  cardiometabolic condition management including disease process, treatment modalities, medication management and lifestyle modification as it pertains to hypertension, hyperlipidemia,  and primary care management of coronary artery disease and diabetes. </li><li>You are experienced working on an interprofessional team and enjoy team-based care.</li><li>You have great clinical and non-clinical judgment.</li><li>You are thorough and take the time to address the needs of your patients. </li><li>You are deeply empathetic and humanistic, and want to go the last mile for your patients.</li><li>You enjoy a fast-paced, high-energy, organization. You thrive on knowing your work can help make these technologies better for you and your patients.</li><li>You learn from every experience and are not afraid to fail - thats how youre wired.</li><li>Finally and most importantly, you have a passion for making healthcare better, solving complex problems, and supporting the delivery of healthcare that we would want for our own family members.</li></ul>Desired skills and experience:<ul><li>APRN with 3 or more years working in outpatient clinical practice ideally with experience in management of hypertension, hyperlipidemia and primary and secondary prevention of atherosclerotic cardiovascular disease. </li><li>Minimum of 2 years of experience concentrated in primary care or a subspecialty with heavy focus on hypertension and lipid management  required (eg. cardiology, nephrology, neurology, endocrinology, primary care, urgent care). </li><li>Proficiency in using telehealth technology and electronic health records (EHR).</li><li>Virtual care experience is preferred along with a strong desire to continue practicing clinical nursing and performing virtual visits - you believe in the mission of bringing care to where the patient lives. </li><li>An understanding of managed care is a plus, including how to appropriately assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps. </li><li>Proficiency in English and Spanish preferred for this position.</li><li>Multi-state licensure is required in addition to a willingness to obtain, and maintain, additional licensure as requested.</li></ul>Licensure and Certification:<ul><li>Masters or Doctoral degree in Nursing; specialization in primary care, internal medicine or geriatric medicine preferred</li><li>An active and clear RN and APRN license in the state of [MARKET] as well as APRN certification is required at time of hire and must be maintained while employed at Devoted Medical.</li><li>Willingness to obtain and maintain multiple state licenses. </li><li>Active BLS is required at time of hire and must be maintained while employed at Devoted Medical</li></ul>Salary Range: $115,000-$150,000 annually#LI-Remote<ul></ul>Our ranges are purposefully broad to allow for growth within the role over time.
