This role plays a critical part in helping our members successfully recover after a hospital or skilled nursing facility stay. You’ll coordinate care, connect members with resources, and ensure a smooth transition from facility to home—all while working remotely.</div><div>You’ll primarily interact with members and providers by phone, ensuring patients have the support they need to stay healthy and avoid unnecessary readmissions.</div><div><b>What You’ll Do</b></div><ul><li><p>Create personalized post-discharge care plans</p></li><li><div>Coordinate follow-up care, services, and resources</div></li><li><p>Educate members and families about medications, appointments, and care instructions</p></li><li><p>Help navigate questions about insurance, eligibility, or care needs</p></li><li><p>Collaborate with hospitals, home health agencies, and other care providers</p></li><li><p>Maintain accurate documentation of all care coordination activities</p></li><li><p>Participate in internal team meetings and quality improvement efforts</p></li></ul><div></div><div><b>Who You Are</b></div><ul><li><div>Licensed RN in California (required)</div></li><li><div>2+ years of care management experience (required)</div></li><li><div>Experience in Medicare Managed Care or similar setting</div></li><li><div>Strong communication, time management, and problem-solving skills</div></li><li><div>Comfortable working independently in a fast-paced, remote environment</div></li><li><div>Willing to obtain licensure in additional states (company reimburses fees)</div></li></ul><div><b>Preferred: </b></div><ul><li><div>Case Management Certification (CCM or equivalent)</div></li><li><div>Knowledge of HEDIS and Risk Adjustment programs</div></li></ul><div><b>Work Schedule</b></div><ul><li><div>Monday–Friday | 8:00 AM – 5:00 PM PT</div></li><li><div>Fully Remote (Work from home)</div></li></ul><div><b>Why This Role Matters</b></div><div>This is a temporary position helping fill a critical gap in our TOC team.
