At Aledade, a PTS can impact the health of their community by transforming one practice at a time.</div><div><br /></div><div><b>Please note: candidates must be based in California to be considered.</b></div><div><br /></div><div><br /></div><div><br /></div><div><b>Primary Duties:</b></div><h3>Direct Practice Support: </h3><ul><li>Implement products and execute ACO initiatives with direction from Market Leadership.</li><li>Accountable for implementing and supporting wellness visits, transitions of care, active management and ensuring practices meet goals set forth in mutually developed Practice Transformation Plans.</li></ul><h3>ACO Training &amp; Development:</h3><ul><li>Work to assist in training office staff and physicians &amp; providers (in coordination with Aledades Value Based Care Curriculum) on processes and technologies.</li><li>Provide continual assessment of practice’s and staff’s training needs. </li><li>Develop and nurture lasting relationships and engagement with physicians, providers, and healthcare system executives to support and enhance financial and quality performance across performance metrics.</li></ul><h3>Customer Success:</h3><ul><li>Listening to practices concerns, relaying important information to Manager/Leads to understand opportunities for improvement.</li></ul><h3>Minimum Qualifications:</h3><ul><li>Must be resident of California</li><li>Graduate of an accredited college or university. 4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting.</li><li>3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes</li></ul><h3>Preferred KSAs:</h3><ul><li>Excellent oral and written communication skills (i.e., public presentation skills), organizational and project management skills, and team-building skills</li><li>Familiarity with process mapping and work-flow analysis tools</li><li>Excellent computer skills and willingness to learn additional software applications</li></ul><h3>Physical Requirements:</h3><ul><li>Sitting for prolonged periods of time</li><li>Extensive use of computers and keyboard</li><li>Occasional walking and lifting may be required</li><li><b>Ability to travel to practices up to 50% in Southern California</b></li></ul><div><b>Who We Are:</b></div><div>Aledade, a public benefit corporation, exists to empower the most transformational part of our health care landscape - independent primary care.
