Major Responsibilities:

  • Provides coordination of care by ensuring smooth transitions between services provided including planning and scheduling of the clinical itinerary which includes pre and post visit activities, escorting patients during visits, and ensuring timeliness of testing and diagnostic reports.
  • Supports the patient and family by providing education, advocating, providing coordination of care and identifying barriers to care. Navigates patient and family members through the coordination of care including coordinating services, treatments and ensuring follow-up of the patient.
  • Develops a relationship with patient and their multidisciplinary team. Ensures quality of patient services and overall patient satisfaction. Addresses and resolves patient questions, concerns, and/or complaints.
  • Facilitates appropriate and prompt access to physician specialists and tertiary care, and coordinates patient information and communication between the referring/accepting facilities and physicians as well as patient referrals to specialized services.
  • Collaborates with the providers to let them know of any clinical concerns the patient may have and to anticipate special needs resulting from the patients clinical care.
  • Collaborates with the international and executive health patient program team, environmental services, food services, nurse managers, physicians, and staff in coordinating the level of services to be provided before, during and after visit of patients.
  • Maintains continuous contact with patient and family during the visit to ensure that the patient is receiving services as requested, to ascertain any additional needs and to assure delivery of service.
  • Provides information for and/or arranges accommodations, transportation, parking, limousine service, and other services (i.e. salon, massage) for patients and families, and arranges availability of special equipment to meet the needs of patients. Ensures coordination with participating hotel partners to arrange and ensure a seamless stay for patients and families and address any special needs.
  • Communicates and consults with business services coordinator during any patient visit to ascertain and verify patient services provided as well as help coordinate payment for services. Ensures documentation of services are completed and collected with billing encounter forms for post-visit activities.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs, and to provide the care needed as described in the departments policies and procedures. Age-specific information is developed further in the departmental job standards.


Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices.


Education Required:

  • Associates Degree in Nursing.


Experience Required:

  • Typically requires 5 years of experience in clinical decision making and coordination of care including experience in providing exceptional hospitality and patient service.


Knowledge, Skills & Abilities Required:

  • Working knowledge of the components of quality and acute patient care needs.
  • Maintains clinical expertise to effectively manage physical, emotional, psychosocial and spiritual needs of patients.
  • Excellent communication and interpersonal skills. Demonstrated ability to work well with and build relationships with a varied and diverse population including physicians and other professionals in a direct and positive manner.
  • Excellent analytical and interpersonal communication skills necessary to negotiate with families, patient, physicians and third party payers.
  • Excellent organization, prioritization and time management skills. Ability to handle multiple and unexpected tasks, set appropriate priorities and accomplish assignments in a thorough and timely manner.
  • Must be detail-oriented with a high degree of accuracy.
  • Intermediate computer skills.


Physical Requirements and Working Conditions:

  • Exposed to a clinical environment.
  • Must be able to sit, stand, walk, lift, squat, bend, twist, and reach above shoulders frequently throughout the workday.
  • Must be able to:
  • - lift up to 50 lbs. from floor to waist.
  • - lift up to 20 lbs. over the head.
  • - carry up to 40 lbs. a reasonable distance.
  • Must be able to:
  • - push/pull with 30 lbs. of force.
  • - perform a sliding transfer of 150 lbs. with a second person present.
  • May be exposed to mechanical, electrical, chemical, and radiation hazards as well as blood and body fluids; therefore, personal protective equipment must be worn as necessary.
  • Position requires travel; may be exposed to road and weather hazards.
  • Must have clear speech, vision and hearing.
  • Operates all equipment necessary to perform the job.

Pay Range

$37.50 - $56.25
