Hybrid Role (Part onsite, part remote). Monday-Friday with typical hours of 8:30am-5pm.
Major Responsibilities:
- Closely collaborate with primary care clinicians in the medical group setting to coordinate patient physical and mental health care.
- Screen, assess, and provide psychoeducation to patients for common mental health disorders. Facilitate patient engagement and follow-up care for a designated case load.
- Create a treatment plan and provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
- Systematically track and document treatment response and monitor patients (in person or by video/telephone) for changes in clinical symptoms and treatment side effects or complications. Track patient follow up and clinical outcomes using a registry, in addition to documenting in the electronic medical record.
- Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
- Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Licensure, Registration, and/or Certification Required:
- License not required but preferred. (e.g. LCSW, LMFT, LCPC, Licensed Clinical Psychologist)
Education Required:
- Masters Degree Social Work, Counseling, Marriage and Family Counseling, Psychology, Nursing.
Experience Required:
- Minimum 1 year experience, behavioral or healthcare experience preferred
Knowledge, Skills & Abilities Required:
- Effective organizational and time management skills
- Competent technology skills, with the ability to work with patients via telehealth and/or in person
- Demonstrate ability to collaborate and communicate effectively in a team setting
- Knowledge of DSM-5 and experience with screening for common mental health diagnoses
- Knowledge of state and federal laws regarding confidentiality, mental health codes, and substance abuse codes
- Experience and comfort in regular interactions and professional dialogue with clinicians
- Experience with assessment and treatment planning for common mental health and/or substance use disorder
